What are the clusters of s/s that would lead you to perform the ACL tests?

- hx macrotrauma (includes twisting and deceleration)
- hx giving way
- hx of hearing "pop"
- intra-articular effusion

What does the anterior drawer test assess?

anterior medial band of ACL

anterior drawer test position

- same as posterior drawer
- pulling tibia anteriorly instead of pushing
- keep elbows straight and rock back

Lachman's test pt position

20-30˚ flexion

What is a positive Lachman's test?

> 3 mm translation and an empty end feel or lack of capsular end feel

What should you find with a normal Lachman's test?

feel a clunk where the ACL stops the tibia from moving

What does the pivot shift test assess?

- ACL
- middle 1/3 of lateral capsule

pivot shift test procedure

- hand under heel
- flex hip to 50-60˚
- hand on lateral joint line
- give valgus force while flexing the knee

pivot shift test hand position on joint line

- thumb under fibular head
- fingertips toward ceiling

What is indicative of a positive pivot shift test?

- full extension: reduced
- 30˚ flexion: pivots
- 60-90˚ flexion: reduces back

What does it mean if you get a pivot at 30˚ flexion on the pivot shift test?

- all 3 bands should be taut, but aren't
- allows tibia to pivot/rotate/shift on the femur

What does the jerk test assess?

- ACL
- middle 1/3 of lateral capsule

What is the jerk test procedure?

same as pivot shift test, but moving from flexion to extension

FRD

flexion rotation drawer

What does the FRD test assess?

- ACL
- middle 1/3 of lateral capsule

FRD procedure

- hug ankle under arm
- grab proximal tibia with both hands
- apply valgus force and gentle compression while moving from extension to flexion

What is happening to the patella in the FRD?

moving the patella to contralateral ASIS

What type of test is the FRD?

2 planar

should see nice, smooth motion

- subluxation of femur on fixed tibia
- wiggle/wobble
