What is shock?

Acute circulatory failure leading to inadequate tissue perfusion with oxygen and nutrients

What is CARDIO OUTPUT?

HR x SV

How is BP calculated?

CO x TPR

Venous return and cardiac function

So if shock = lack of blood pressure, then what can shock be classified into?

CO = HR x SV - Cardiogenic
TPR - Distributive
SV -Hypovolaemic
Lastly, Obstructive

What are the causes of cardiogenic shock?

Depressed contractiliy - AMI, Cardiomyopthay, myocarditis, drug toxicity
Acute valve dysfxn - rupture
Arrhythmias - too slow/too fast to support CO

What are the causes of hypovolaemic shock?

Decreased venous return due to inadquate intravascular volume so
Haemorrhage, GI losses, Burns

What are the causes of distributive shock?

Anaphylaxis, sepsis, drug toxicity

Obstructive shock

Decreased venous return - PE, Tension pneumo, cardiac tamponade

Principles of treatment

Monitor
Oxygenation/ ventilation
Optimize heart and rhythm
Optimize preload and Hb
Optimize afterload
Optimize cardiac function
