Ketoconazole- Uses

Systemic treatment of mycoses- Histo, blast, candida, coccidiodes

Ketoconazole- MOA

Inhibits sterol in fungal membranes- inhibits cell growth, structure and function

Ketoconazole- AE

GI upset, nausea, anorexia & vomiting.
Gynecomastia & decreased libido

Ketoconazole- Interactions

potentiate toxicity of warfarin, shorten duration of other azoles. Teratogenic.

Imidazoles- keton, micon & clotrimazole- Indication

Topical agents- epidermophytom, microsporum, candida albicans. Contact dermatitis, vulvar irritation & edema

Imidazoles- SE

Bleeding with warfarin

Fluconazole- Indications

Bone marrow prophylaxis, cryptococcus, vaginal candiaiasis. Excellent permeability into CSF and lack of endocrine side effects

Fluconazole- MOA

Inhibits synthesis of fungal membrane

Fluconazole- Se

N/V/rash. Caution with liver disfunction. Teratogenic.

Itraconazole- Indications

Broad spectrum. DOC- Blasto, spototrichosis, histo, coccidio. Not absorbed by CSF

Inhibits synthesis of fungal membrane

Itraconazole- SE

Inhibits other drugs metabolism- CYP 450. N/V/rash hypokalemia, hypertension & edema. Pregnancy, CHF

Griseofulvin- Indication

dermatophytic infections of nails, skin & hair- ringworm & dermaphytosis. Requires 6-12 months tx

Inhibition of fungal mitosis

Griseofulvin- SE

Increases rate of anticoagulant metabolism

Terbinafine- Indications

DOC- onchyomycoses & dermatophytoses. Fungicidal- candidas & tineas

Terbinafine- MOA

Inhibits fungal squalene epoxidase- leading to cell death

Terbinafine- SE

GI, HA, rash, taste & visual disturbances
