What is appendicitis?
Inflammation of the appendix, usually due to obstruction, leading to infection and possible perforation
What are the classic symptoms of acute appendicitis?
Periumbilical pain migrating to right lower quadrant, anorexia, nausea, vomiting, and fever
What are important signs to look for on examination in suspected appendicitis?
Right iliac fossa tenderness, rebound tenderness, guarding, and possible low-grade fever
What imaging is first-line in suspected appendicitis?
Ultrasound abdo / pelvis
what is the gold standard imaging in diagnosing appendicitis?
contrast CT abdo/pelvis
What are complications of untreated or delayed appendicitis?
Perforation, abscess, peritonitis, and sepsis
What is the management if an appendix mass or abscess is found?
Percutaneous drainage and antibiotics may be considered, with delayed surgery if needed
Where is the McBurney’s point?
1/3 of the distance from the anterior superior iliac spine (ASIS) to the umbilicus, in the right lower quadrant
Clinical relevance of the McBurney’s point
Tenderness at this point, and Rebound tenderness = appendicitis
What is Rovsing’s sign?
Palpation of the left lower quadrant causes pain in the right lower quadrant, due to peritoneal irritation
What is the psoas sign?
Pain on extension of the right hip, indicating irritation of the psoas muscle by an inflamed retrocaecal appendix
What is the obturator sign?
Pain on internal rotation of the flexed right hip, suggesting a pelvic appendix irritating the obturator internus muscle
What is the significance of a palpable mass in the right lower quadrant?
May indicate an appendiceal mass or abscess formation in complicated appendicitis
Classic Appendicitis Sx sequence: essential for exam
Anorexia (loss of appetite), then pain, then vomit
Appediceal abscess Sx
- Develops RIF pain and fever for 1-2 days then resolves, then recurs 7-10 days later
- Palpable mass on RIF
Appendicitis Mx
- Open Appendectomy: McBurney (oblique) or Rocky-Davis (transverse) right lower quadrant muscle-splitting incision
- Laparoscopic appendectomy
What is the role of antibiotics in appendicitis management?
IV antibiotics should be given pre-operatively and post-operatively
Most common post appendectomy complications
- Surgical Site Infection
- Intra-abdominal or Pelvic Abscess
