Flashcards in Arrhythmias Deck (21):
1

What are ectopic beats?

Beats or rhythms originating in places other than the SA node

2

Name three supraventricular tachycardias

Atrial Fibrillation

Atrial Flutter

Ectopic atrial tachycardia

3

What type of arrhythmia is Wolfe-Parkinson-White?

Accessory pathway atrio-ventricular node arrhythmia

4

What type of arrhythmia are AV blocks?

Atrio-ventricular node arrhythmias

5

Name some ventricular arrhythmias

Ventricular tachycardia

Ventricular Fibrillation

Asystole

6

What are common symptoms of arrhythmias?

Palpitations

Dyspnoea

(Pre)syncope

Sudden cardiac death

Worsening of pre-existing conditions - angina, CHF

7

How are arrhythmias investigated?

12 lead ECG

CXR

Echocardiogram

Stress ECG

24 hour Holter monitoring

Event recorder

Electrophysiological study - induction of arrhythmia to study

8

What are the symptoms of atrial ectopic beats.

What is the management?

Asymptomatic/Palpitations

Generally no treatment/beta-blockers

Avoidance of stimulants

9

When would pacing be used as a treatment for sinus bradycardia?

If the patient is haemodynamically compromised:

hypotension

CHF

angina

collapse

10

What is the pharmaceutical treatment for acute sinus bradycardia (e.g. MI)

Atropine

11

In what kind of MI is sinus bradycardia the most common?

Inferior STEMI

12

What is the treatment for sinus tachycardia?

Treat the underlying cause

beta-blockers

13

What is the acute management for supra-ventricular tachycardia?

Vagal maneouvres, carotid massage

IV adenosine

IV verapamil

14

What is the chronic management for supra-ventricular tachycardia?

Avoidance of stimulants

Ablation

Antiarrhythmic drugs (class II or IV)

15

What is 1st degree heart block?

What is the management?

Long PR interval ( >0.2s)

Long term follow up to monitor for development of more advanced block

16

What is Mobitz Type I 2nd degree block?

What is the usual origin?

Progressive lengthening of PR interval eventually resulting in dropped beat.

Usually vagal in origin.

17

What is Mobitz Type II heart block?

What is the management?

Intermittent dropped beats due to block at AVN

Pathological and may develop to complete heart block.

Permanent pacemaker

18

What is 3rd degree block?

What is the management?

Complete block at AV node - no link between atria and ventricles.

Pacemaker

19

Give examples of inherited ventricular arrhythmias

Long QT

Brugada syndrom

20

What is the long term treatment for VT?

Correct ischaemia (revascularization)

ICD if life threatening

Optomise therapy for CHF
