45 yo male complains of headache behind the eyes that sometimes radiates to the vertex. You notice his eyes and face are quite red and he has to constantly drink from his water bottle. When you ask about sleep he admits to insomnia with nightmares and says he will also occasionally wake with nosebleeds.

Pulse is wiry, rapid and full.
