ACTIONS

A

Decreases gastrointestinal motility.
Inhibits sweat and salivary glands.
Inhibits glandular secretions of the GI tract.
Relaxes bronchial smooth muscle - bronchodilation.
Acts as an acetylcholine antagonist​ by competitively binding to muscarinic receptors on exocrine gland, smooth muscle, and cardiac tissue.
Blocks parasympathetic innervation of the heart increasing SA node automaticity and AV node conductivity.

Indications

A

Symptomatic bradyarrhythmias
CPA with suspected increase in vagal tone
Organophosphate / cholinergic agent exposure
Status asthmaticus
Symptomatic conduction deficits- ie Stokes Adams.

Precautions

A

Has been shown to be harmful in some blocks below the AV node at the purkinje/bundle of HIS level (new wide complex 2nd or 3rd degree blocks). Atropine may be used in these situations but monitor closely for paradoxical slowing
Physostigmine must be administered as antidote
Too low of a dose may cause paradoxical bradycardia.
may worsen glaucoma and myasthenia gravis
May exacerbate urinary retention in males with enlarged prostates.
May increase myocardial O2 demand in MI pts and aggravate chest pain.
