malignancy (most commonly mets and myeloma)
hyperparathyroid
thyrotoxicosis
excessive intake
sarcoid
TB

What tests do we do for hypercalcaemia?

ECG
look for cancer
Blood: ca, Ig's. serum free light chains
bence jones
PTH
ESR

How do we treat hypercalcaemia??

Nacl over 4hrs
if no better after 48hrs then give a single zolendronic acid infusion

What causes hypocalcaemia?

lack of vitamin D
chronic renal failure
hypoparathyroid
bisphosphonates
low plasma albumin
low diet

How does hypocalcaemia present?

parasthesia
tetany
cramps
confusion
seizures
long QT

How do we treat hypocalcaemia?

oral or IV calcium

Why is calcium adjusted with albumin?

Ca binds to albumin.

When do we give IV calcium for hypocalcaemia?
What do we give?

if tetany
calcium gloconate 10ml, 10% over 10 mins
then a 40ml infusion
