What is the initial assessment of critically ill patient?

DR ABCDE

Danger

Ensure patient/staff safe
Check for other dangers
Ensure protective equipment i.e. gloves

Assess responsiveness
Call patient name/observe

Airway

Patency, obstruction, protective reflexes
Jaw thrust, Chin lift

Breathing

Work and efficacy of breathing

Circulation

Shock, unstable arrhythmias

Disability

GCS - <8

Environment, Exposure, Examination

Temp, blood glucose, secondary survey

Airway failure

Open and clear airway: prevent aspiration

Resp failure

O2, consider assissted ventilation

Circulatory failure

ECG, BP, Pulse oximeter, IV access

Supine position
Precordial thump
Head tilt/Chin lift/ Jaw thrust
Airway
Ventilations
Compression 30/2
Defib

Steps in ALS

Defib only for VFib or Vtach
Attach, check, shock
Recommence CPR
Monitor after 2 minutes
IV access - adrenaline 1 mg IV
Give amiodarone if no response

If a systole: what do?

Commence CPR for 2 mins
Give adrenaline 1 mg
Atropine 1 mg
Recheck rhythm

What are the main causes of cardiac arrest?

4Hs and 4Ts

What are the 4H

Hypothermia
Hypovolaemia (bleed!!)
Hypoxaemia
Hyper/hypokalaemia, metabolic disorders

What are the 4Ts

Tension pneumothorax
Tamponade
Toxins
Thromboembolism

What are the post-resus care principles

RR support
Main cerebral perfusion - >100mmHg
Avoid metabolic issues
Treat underlying cause
Transfer to ICU
