  • HGPRT deficiency
  • under excretion of uric acid
  • renal impairment

  • drugs
  • exercise, dehydration, starvation
  • lead poisoning

What is the managment of gout?

A

Acute flare:

  • NSAIDs
  • colchicine
  • steroids

Hyperuricaemia:

  • xanthine oxidase inhibitor
  • febuxostat
  • uricosuric agents (e.g. sulphinpryazone)
  • canakinumab

* treat if 2nd attack within 1 year

* prophylactically

  • anaemia
  • weight loss, malaise, depression
  • fever
  • arthralgia, synovitis
  • ESR > 45 (often 100)

  • prednisolone (15mg/day, initially)
  • 18-24 month course
