S/S of Major Depressive Disorder

5 or more of the following have been present in same 2 week period.
Depressed mood
Diminished interest in activities
weight loss or gain
insomnia or hypersomnia
fatigue, loss of energy
suicidal ideations
feeling of worthlessness
inability to concentrate or make a decision
Psychomotor agitation or retardation

Lack of interest in activities formerly enjoyed

Dysthymic Disorder

A chronic mood disturbance with S/S of insomnia, loss of appetite, decreased energy, low self esteem, diff concentrating, and feeling sadness and hopelessness. Depressed for most days of the year.

unrelenting, rapid, often loud talking without pause

fast speech, often unconnected thoughts

Cyclothymic disorder

Mild mood swings between hypomania and depression without loss of social or occupational function

Catatonic: Patient is immobile or unresponsive (stupor), mute
Mealancholic: characterized be sadness, crying, despondence (loss of hope or courage)

Name SSRI (Selective Serotonin Reuptake Inhibitor) and their side effects

End in -ine and -pram
S/E: have GI (N/V, diarrhea, constipation), nervousness, drowsiness), and sexual side effects
Fluoxetine
More stimulating - Sertraline, Citalopram
Fewer drug-drug inx - Escitalopram
Paroxetine - more sedating

Norephinephrine -Serotoine reuptake inhibitor (NSRI)

Venlafaxine - Strong antidepressant
Duloxetine - Good for neuropathic pain including Fibromyalgia, as well as depression

MAO (Monoamine Oxidase) inhibitors Side effects

Special diet (Tyramine free) many drug-drug inx, can lead to extreme HTN

TCA (Tetracyclic Antidepressant) Side effects

Older meds, work on Serotonin and Norepinephrine, many side-effects, lethal in overdose
S/E: tardive dyskinesia, extrapyramidal symptoms, neuroleptic malignant syndrome.

Miscellaneous medications and their S/E

Bupropion (no sexual side-effects) can cause seizures
Trazodone: used for sleep, can cause priapism (persistent and painful erection of the penis)

Nursing care for Electroconvulsant Therapy (ECT)

Can be used for pregnant women
NPO after midnight
Remove fingernails
Patient is not awake during the procedure
Seizure precaution
Return of gag reflex is assessed

Suicidal talk
Death preoccupations
Behavioral change/ sign of depression
Giving things away
Appetite/ sleep disturbance
Taking excessive risk. increasing drug use
