Flashcards in Diagnostic Testing Deck (20):
Is an electrocardiogram specific for congestive heart failure⁉️
No it is nonspecific
How can an electrocardiogram assist in diagnosing CHF⁉️
It can suggest prior MI, conduction system disease, and chamber enlargement
How can chest radiograph assist in diagnosing CHF patient⁉️
May show cardiomegaly and signs of pulmonary congestion
What is the relationship between clinical improvement and chest radiograph improvement in a patient who has received treatment for a chat?
Clinical improvement will happen 24 to 48 hours before chest radiograph changes may occur
What test is recommended for all patients suspected of HF⁉️
Transthoracic echocardiographs (TTE)
What characteristics of the heart can TTE assess⁉️
Ventricular chamber size, ventricular wall thickness, systolic function, diastolic function, and valvular stenosis or regurgitation
What quantification is it able to acquire?
Left and right atrial pressure's, stroke volume, cardiac output
What test should be done to assess the efficacy of therapies in patients who have HF?
What initial laboratory and valuation should be done for CHF patients? Why⁉️
Complete blood count (CBC) for anemia. Basic chemistry panel (BCP) for electrolyte abnormalities
What electrolyte animality is common in CHF patients?
Serum sodium level impairment
Signs of what organ dysfunction may occur in CHF patients⁉️
Kidney and liver
What would be the cause of renal dysfunction in CHF patients?
Decreased cardiac output and renal artery vasoconstriction or elevated venous pressures reflected in Renal veins
What should HF patients be evaluated for? Why?
Hemachromatosis, hypothyroidism, hypothyroidism; because these are reversible causes of HF
What serum level can diagnose hemachromatosis?
Serum ferritin level
What are the laboratory test for modifiable risk factors in HF?
Fasting lipid panel, blood glucose level
What virus should HF Patients be tested for?
Increase in liver enzymes do to have had a congestion may result from what?
Left and or right ventricular HF or severe tricuspid regurgitation
What can be measured in patients who have dyspnea with the diagnosis of HF doubtful?
BNP, natriuretic peptide levels
What can we determine if serum BNP results are normal⁉️
Strong discriminatory factor to eliminate HF as cause of dyspnea
