Flashcards in Disease & Associated antigens/antibodies Deck (34):
1

Churg-Strauss OR Microscopic Polyarteritis

pANCA

1

Major Sx for Henoch-Schloen?

Henoch

  • J - Joints
  • A - Abdominal pain
  • R - Renal
  • S - Skin

2

RA (2)

HLA-DR4

ACPA

3

Polymyositis (2)

anti-Jo1  (anti-tRNA synthetase)

Anti-SRP

3

Evaulate this ANA (4)

Speckled

  1. CREST/Scleroderma (anti-centromere)
  2. Neonatal Lupus (due to Subacute Lupus) (Anti-Ro/SSA)
  3. SLE (Anti-Smith)
  4. Sjogren's (Anti-Ro/SSA, Anti-La/SSB)

*remember that this is the most common pattern, but is also the least specific* BUT NOTE THEY ALL START WITH S = S FOR SPECKLED

If you get speckeled pattern, test further for

  • Anti-smith (SLE)
  • Anti-Topo I (Scleroderma-diffuse)
  • Anti-La (Sjogrens, SLE)

also - connective tissue disease" (MCTD) - "overlap" condition that is a mix among SLE, scleroderma, and polymyositis features, but without serious renal or pulmonary disease. We were never taught this so w/e.

4

SLE ANA

peripheral pattern

Anti-smith

4

3 Diagnostic Tests for Fibromyalgia

CBC

ESR

TSH

5

Henoch Schoenlen Purpura

IgA

5

SLE markers(2)

ANA pattern?

Anti-dsDNA - highly associated with GN in SLE

Anti-Smith - VERY SPECIFIC for SLE

peripheral "rim" pattern

7

Marker for:

JIA - Pauci-articular early onset

JIA - Pauci-articular later onset

What does it each indicate?

early onset: ANA (+) - chronic uveitis

later onset: HLA-B27 (+) - acute uveitis, lower limb enthesis and/or sacroiliitis

8

ANA (+), (3)

Myopathies (Dermatomyositis, Polymyositis)

Juvenile Idiopathic Arthritis, pauci, early onset; indicates increased likelihood of anterior uveitis

9

Raynaud's Phenomenon - typically associated with what?

High ANA titer (>160), specifically anti-centromere due to CREST (Scleroderma)

11

ANA for CREST

anti-centromere

12

Lupus Nephritis

Anti-DNA

12

Ca pyrophosphate dihydrate crystals (CPPD)

Type of Crystals and association

blue when parallel to the light

yellow when perpendicular to light

(+) birefringence

rhomboid shape

CHONDROCALCINOSIS

13

Evaulate this ANA.

Rim "Peripheral"

SLE

14

urate crystals

yeLLow when paraLLel to the light

bLue when perpendicular to the light

(-) birefringence

pencil shape

15

Dermatomyositis (2)

anti-Jo1

anti-Mi2

16

Marker for:

JIA - systemic

elevated ESR, CRP

RF (-)

17

Wegener's "Granulomatosis w. Polyangiitis" GPA

cANCA

19

Marker for:

JIA - OLIGO-articular early onset

JIA - OLIGO-articular later onset

What does it each indicate?

early: no specific marker, but tenosynovitis

late: RF(+), early erosions, joint destructions, poor functional outcomes; treat with DMARDs early

21

ANA for drug-induced lupus

Anti-histone

22

Osteogenica Imperfecta

COLIA1, COLIA2 (Type I collagen)

23

Scleroderma - limited form (2)

Anti-topoisomerase II

Anti-centromere (CREST)

24

Present in both Polymyositis and Dermatomyositis

anti-Jo1

25

Sjogren's Syndrome (2)

Anti-Ro/SSA

Anti-La/SSB

26

elevated CPK (4)

Rhabdomyolysis

Drug-induced myopathy (statins only; steroids causes normal CPK)

Polymyositis

Dermatomyositis

27

Major Sx for kawasaki

  • kawasaki dz:
  • children = coronary artery aneurysm
  • really = rash (desquamting)
  • like = lymphadenopathy of cervical nodes
  • eating = eye (conjunctivitis)
  • fresh = fever
  • strawberries = tongue

28

HLA-B27 (3)

  1. SpA
  2. Reactive Arthritis
  3. Juvenile Idiopathic Arthritis - pauci form, late onset (50% of these patients develop SpA

29

RF (5)

  1. RA
  2. elderly
  3. bacterial endocarditis
