Dopamine - Action (5)

A
  • Dose dependent sympathomimemtic
  • 1-2 mcg/kg/mins stimulate dopaminergic receptors > cerebral/renal/mesenteric vasodilation
  • 2-10 mcg/kg/mins B1 effects (mainly inotropic) w/ alpha beginning (vasoconstriction)
  • 10-20 mcg/kg/mins alpha effects ^renal/mesenteric/peripheral arterial vasoconstriction (increased SVR)
  • 20+ mcg/kg/mins - alpha effects similar to norepi w/ peripheral vasoconstriction/tachycardia

HUMP

A
  • Hypovolemia prior to fluid resuscitation
  • Uncorrected tachyarrhythmias
  • Monamine oxidase Inhibitors (MOI)
  • Pheochromocytoma (precipate HTN crisis)

Dopamine - Precautions (4)

A
  • Monitor VS/EKG
  • Stop infusion if BP >90 mmHg
  • Doesn’t work w/ Bicarb due to Alkaline deactivating dopamine
  • avoid extravasation => tissue necrosis, inform RN
