describe the thyroid

butterfly like, isthmus connects. Develops from pharynx, can have topic regions. End result is increased T4 and increased basal metabolic rate

check TSH changes- will see first!

endogenous autoimmune disorder against TSH receptors. lots of lymphocytes, tall/columnar epithelium, enlarged gland. elevated T3/T4 with decreased TSH

hypothyroidism of adults- mental sluggish, cold, obese, low BM

diagnosing hypothyroidism

T4 always decreased. TSH elevated in primary hypothyroidism

hashimotos

goiter, autoimmune disease. mononuclear infiltrate with lymphoid follicles. Parathyroid is OK. enlargement painless, can remove

goiters in general

systemic enlargement. low t4/T3 but high TSH

adenomas

solid, benign, can become malignant, cold, well intact capsule/well differentiated

papillary thyroid cancer

MAP kinase inactivation, distinct from medullary. dense fibrovascular core with cervical lymph nodes. Pale empty nuclei!
