Temperature > 38.5 (or 101.5)
What are the 5 classic Ws of postoperative fever?
Wound (wound infection)
Wonder drugs (drug fever)
What is the classic timing of postoperative atelectasis?
First 24-48 hours
What is the classic timing of postoperative UTI?
Anytime after POD #3
What is the classic timing of postoperative wound infection?
Usually after POD #5 (but it can be anytime)
What is the classic timing of postoperative DVT/PE/thrombophlebitis?

What is the most common cause of fever on PODs #1-2?
What is the complete fever workup?
Physical exam, CXR, U/A, blood cultures, CBC
What causes fever before 24 postoperative hours?
Atelectasis, B-hemolytic streptococcal or clostridial wound infections, anastomotic leak
What causes fever from POD #3-5?
UTI, pneumonia, IV site infection, wound infection
What is an anesthetic cause of fever intraoperatively?
Malignant hyperthermia (treat with dantrolene)
What causes fever from POD #5-10?
Wound infection, pneumonia, abscess, infected hematoma, C. difficile colitis, anastomotic leak, DVT, peritoneal abscess, drug fever, PE, parotitis
What causes wound infection on POD #1-2?
Streptococcus or Clostridium
