Class

  • Loop diuretic
  • (For high pressure cardiac pulmonary edema)

Mechanism of action

  • Inhibits electrolyte reabsorption
  • Promotes excretion of sodium, potassium, and chloride

Indications

  • CHF
  • Pulmonary edema
  • Hypertension crisis

Contraindications

  • Hypovolemia
  • Hypotension (relative contraindication)
  • Allergic
  • Anuria
  • Hepatic coma

Adverse reactions

  • May exacerbate:
  • hypovolemia
  • hypokalemia
  • hypochloremia
  • hyponatremia
  • ECG changes
  • dry mouth
  • hyperglycemia (due to hemoconcentration)

Drug interactions

  • Lithium toxicity may be potentiated by sodium depletion
  • Digitalis toxicity may be potentiated by potassium depletion

Dosage and administration

  • Adult:
    • 0.5 - 1.0 mg/kg injected IV over 1-2 minutes
    • If no response, double the dose to 2 mg/kg 
  • Pediatric:
    • 1 mg/kg/dose IV, IO

Duration of action

  • Onset: 5 min
  • Peak effects: 20-60 min
  • Duration: 4-6 hrs

Special considerations

  • Pregnancy: Cagetory C
  • Rapid push can cause ototoxicity and deafness
  • Protect from light
