GI reduced absorption - chronic diarrhoea, insufficient intake
Increased intracellular sequestration - refeeding syndrome, leukemia/lymphoma, DKA recovery, resp alkalosis

Management

A

Treat if moderate/severe
-PO4-sandoz effervescent

Excessive doses => hyperkalemia, hypocalcemia, metastatic calcification

Correct hypocalcemia first
