Borriela burgdorferi. Tick bite in summer, midwest to eastern US

Prodrome-> erythema migrans- central clearing bullseye + myalgia and arthalgia-> secondary lesions, achiness-> musculoskeletal manifestations

Gonococcal arthritis- Eti

Most common in women, during menses or pregnancy. Disseminated gonococci infection. Common in otherwise healthy

Gonococcal arthritis- Sx

polyarthalgias involving wrist, ankle, knee or elbow-> tenosynovitis to wrists, fingers, ankles or toes or purulent mono arthritis involving knee, wrist, ankle or elbow

Gonococcal arthritis- Dx

Urethral, cervical, throat and rectal cultures

Gonococcal arthritis-Tx

Azithromycin + cerftriaxone IV

Viral arthritis- Eti

Caused by many viruses- parvo, Hep B, hep C

polyarthritis, mimics rheumatoid arthritis, self limiting

Bacterial arthritis- Eti

Bacteremia, damaged or prosthetic joints, immunosuppresed, loss of skin. Large joint effusions, monarticular in large weight bearing joints & wrists

Acute onset- pain, heat, swelling, systemic manifestations, frequently knee

Bacterial arthritis- Dx

Aspiration of joint- > 50,000 mcL leukocytes

Bacterial arthritis- Tx

Broad spectrum abx-> ceftriaxone or vancomycin. Surgery for drainage & debridement

Septic bursitis- Eti

Inflammation of bursa due to infection- S. aureus most common.

Septic bursitis- Sx

Focal tenderness, swelling, ROM deficits

Septic bursitis- Dx

Joint aspiration- >50,000 WBC

Septic bursitis- Tx

IV abx, incision and drainage
