What is the pathophysiology of IBS?

What causes the pain in IBS

Infective trigger

What type of pain is found in IBS?

Radiation of central pain - fibromyalgia

What is the criteria for IBS diagnosis?

Pains for 3 months
Improvement with defecation
Change in stool frequency
Change in appearance of stool

What other illness is IBS associated with?

Fibromyalgia
Chronic fatigue syndrome
Joint pain
Chronic pelvic pain
Anxiety

How can IBS be managed?

Diet:
Regular meals
Reduce Fibre

Drugs:
Stop opiate analgesia - important
Anti-diarrhoeals
Anti-spasmodics (Meveberine, Hyoscine)
Anti-depressants

Physcolognical help

What investigations do you do for IBS?

Full blood count - check for anaemia
Plasma Viscosity
CRP marker
TTH - test for coeliac disease
Sigmoidoscopy/ Colonoscopy

How should IBS patients get fibre?

Oats.
Introduce fibre gradually

What drugs should be given for IBS?

Laxatives
Anti-darrhoeals - Loperamide
Anti-spasmodics (Meveberine, Hyoscine)
Anti-depressent - Amitriptyline (reduces afferent signals from the gut, causes resetting of the gut)

What is the side effect of amitriptyline ?

Constipation
