What is IBS?

A mixed group of abdominal symptoms for which no organic cause can be found

* Female sex
* FHx
* Poor coping strategies
* Poor mental health

Symptoms of IBS

Recurrent abdo pain >6m

Associated with at least two of:

* Relief by defacation
* Altered stool form
* Altered bowel frequency

Other features
* Urgency
* Incomplete evacuation
* Abdominal bloating
* Mucus PR
* Sx aggravated by food, stress, menstruation, gastroenteriris (post-infective IBS)

Signs of IBS

May be none

* Generalised abdominal tenderness
* Abdo distention
* Mucus seen on PR

Differential diagnosis of IBS:

* Colonic Cancer
* IBD
* Bile acid malabsorption
* Coeliac disease
* Gastroenteritis
* Diverticular disease
* Gynae problems

Investigations for IBS:

Bloods

Faecal calprotectin

Management of IBS

Constipation
* Inc fluid intake
* Physical activity
* Simple laxatives (avoid lactulose)

Diarrhoea
* Reduce alcohol/caffeine
* Avoid sweetener
* Reduce dietary fibre
* Loperamide + bulking agent
* Identify trigger foods

Colic/Bloating
* Oral antispasmodics (e.g. Hyoscine)
* Probiotics
* Reduce FODMAPs

Psych Sx
* CBT
* Hypnosis
* TCAs (e.g. Amitriptyline)
