Headaches, dizziness, loss of consciousness, visual
problems
What are common symptoms associated with CVS?
chest pain, shortness of breath
What are common symptoms associated with RES?
flu like symptoms, cough
What are common symptoms associated with GI?
Abdo pain, diarrhoea /constipation, blood in stools
What are common symptoms associated with GU?
Dysuria- burning or difficulty when passing urine Hematuria- blood in the urine

Rashes, Joint pain
What are common symptoms associated with any body system?
fever, weight loss, loss of appetite

What existing conditions/ illnesses the patient has.

Any meds? : over the counter meds, herbal remedies
