What is the mechanism of HMG-CoA reductase inhibitors (aka statins)?

  • Inhibit cholesterol precursor, mevalonate production
  • Loss of production forces hepatic cells to absorb more cholesterol

What are the effects of statins on LDL, HDL and TGs?

  • Large decrease in LDL
  • Small inc rease in HDL
  • Small decrease in TG

What  are the side effects of statins?

  • Hepatotoxicity
  • rhabdomyolysis

What is the mechanism of action of niacin?

  • Inhibits lipolysis in adipose tissue
  • Reduces hepatic VLDL secretion into circulation

  • Medium decrease in LDL

What are the side effects of niacin?

  • Red flushed face which is decreased by aspirin
  • Hyperglycemia (acanthosis nigricans)
  • Hyperuricemia (exacerbates gout)

What is the mechanism of action of bile acid resins (cholestyramine, clestipol, colesevelam)?

Prevent intestinal reabsoprtion of bile acids. Liver must use cholesterol to make more bile

  • Medium decrease in LDL
  • Slightly increase HDL
  • slightly increase TGs

What is the side effect of bile acid resins?

  • Patients hate it -- tastes bad and causes GI discomfort
  • Decrease absorption of fat-soluble vitamins
  • Cholesterol gallstons

What is the mechanism of chloesterol absorption blockers (ezetimibe)? 

  • Prevent cholesterol reabsorption at small intestinic brush border

  • Medium decrease in LDL

What is the mechanism of action of "fibrates" (gemfibrozil, clofibrate, bezafibrate, fenofibrate)?

  •  Upregulate LPL causing increased TG clearance
