What are the 4 Sings of pressure?
Stage 1- the skin is still intact- the redness over body prominence colour doesn’t return to normal when pressure is relieved.
Stage 2 of pressure is when Partial thickness skin loss
Skin has cracked and blistered and starts peeling
Stage 3 Full thickness skin loss
An area open and is draining
Stage 4 Full thickness skin loss
Visible you can see bone and the muscle and tendons
Stage X
Appears black and unsure how much damage is done to it
