-longer than 1-2 weeks to resolve
-direct bili greater than 10%
-increase fast

concern for pathologic process

work up for pathologic jaundice

U/S, HIDA scan, sepsis/metabolic panels

elevated unconjugated bili work up

first - coombs test - Rh or ABO
second - look at Hb
-third - check retic count
-all normal - reabsorption problem

tx for elevated bili

bili greater than 10 - UV light
bili greater than 20 - exchange transfusion

breast feeding jaundice

mom not feeding baby enough
-low gut motility
-more reabsorption of unconjugated bili GI tract
-tx - feed baby more

breast milk jaundice

mom's milk inhibit UDP glucuronyl transferase
-more unconjugated bili
-tx - formula while baby clears jaundice
