Risk factors/aetiology of Oesophageal Ca:

* Alcohol
* Smoking (SCC>AC)
* Achalasia
* Reflux oesophagitis (+/- Barrett's)
* Obesity
* Hot drinks
* Nitrosamine exposure
* Plummer-Vinsen syndrome
IDA
Dysphagia
Glossitis
Cheilosis
Oesophageal webs

Anatomical distribution of oesophageal ca:

20% upper 1/3
50% middle 1/3
30% lower 1/3

Symptoms of oesophageal carcinoma:

* Dysphagia
* Odynophagia
* Anorexia
* Vomiting
* Weight loss
* Retrosternal chest pain
* Fatigue, SOB (IDA)
* Melaena
* Haematemesis

If upper 1/3 of oesophagus (20%)
* Hoarseness
* Cough

Signs of oesophageal carcinoma:

* Lymphadenopathy
* Melaena
* Weight loss
* Pallor (IDA)

Differential diagnosis of oesophageal carcinoma:

* Benign peptic stricture
* Bronchial carcinoma
* Paraoesophageal lymphadenopathy
* Gastric cancer
* Achalasia
* Leiomyoma
* Metastatic disease (mainly breast)

Investigations for oesophageal carcinoma:

1. Bloods: FBC (?IDA), U&E, LFTs, Glucose, CRP

2. Urgent OGD: +brushings/biopsies of any lesions

3. CXR: ?Mets

3. CT/MRI TA: For staging

Staging system in oesophageal carcinoma:

TNM

Management of oesophageal carcinoma:

Localised disease
* Radical oesophagectomy
* Pre-op chemotherapy

May be curative


Extensive disease
* Chemoradiotherapy
* Palliation
E.g. to restore swallow

5-year survival of oesophageal Ca:

20-25%
