How can you prevent ERCP-Induced Pancreatitis?
Rectal NSAIDs such as Indomethacin
What are common symptoms of Acute Pancreatitis?
Acute epigastric pain going straight through to the back
tenderness
nausea and vomiting
What is a poor prognostic indicator in Acute pancreatitis?
Low Calcium
What is the best initial test for Acute Pancreatitis?
Amylase and Lipase elevation
What is the most specific test for Acute Pancreatitis?
CT or MRI
What are the treatment options for Acute Pancreatitis?
NPO: high volume IVF
Analgesia
PPI to decrease pancreatic stimulation
If CT/MRI shows >30% necrosis: Imipenem or Meropenem
Surgical resection and debridement for infected necrotic tissue
Needle drainage of Pseudocysts
What is the cause of Autoimmune Pancreatitis?
IgG4 related pancreatitis, ANA and RF (+)
What is the key to diagnosis in Autoimmune Pancreatitis?
Absence of significant alcohol intake or stones
What does biopsy reveal in autoimmune pancreatitis?
Lymphocytic and plasma cell infiltrates
How does symptomatic Autoimmune pancreatitis present?
recurrent Jaundice, weight loss and abdominal pain
Abd CT: enlarged sausage-shaped pancreas
elevated IgG4
How do you treat autoimmune pancreatitis?
Steroids
How does Pancreatic Cancer commonly present?
painless jaundice, weight loss and a generally non-tender epigastric area
What lab values are typically elevated in Pancreatic Cancer?
Bilirubin, Alkaline Phosphatase, GGTP
What is the best initial test for suspected pancreatic cancer?
CT
If a CT scan is Negative for Pancreatic mass what is the next best step?
Endoscopic US with biopsy of lesions
If a CT shows a clear lesion in the pancreas what is the next best step?
Surgical biopsy/removal at the same time
What elevated lab values are associated with Mucinous Cystadenoma and Intraductal Papillary Mucinous Neoplasms of the pancreas?
CEA and CA
What lab follows response to treatment of Pancreatic Cancer?
CA 19-9
