what are the portals of entry into the kidney?

  • haematogenous - localises in large vessels & interstitium; infarction; septic/immune-mediated
  • ascending from LUT - secondary to GIT/dermal/genital contamination; targets tubules & interstitium; more common in females
  • direct penetration - injury within tubules

what are the renal defenec mechanisms?

  • vasculature - BM
  • glomerulus - filtration barrier, glomerular mesangium
  • tubules - BM
  • interstitium - innate humoural & cell-mediated immune responses

what are the renal responses to injury?

  • vasculature - haemorrhage, thrombosis, infarction
  • glomerulus - acute inflammation & hypertropy; atroophy/fibrosis/necrosis of glomerular tufts
  • tubules - cell degeneration (necrosis/apoptosis); cell regneration & renal fibrosis
