Aldosterone ant. block aldosterone - blocking sodium and water reabsorption and K+ excretion
Na+ ion channel inhibitors blocks sodium channel in DCT - inhibiting Na+ reabsorption and K+ excretion
Both are weak diuretics
Indications
HTN
Edema
Heart failure
SE/AE
Hyperkalemia (especially when taking ACEI/ARB)
Men - gynecomastia, impotence, low libido
Women - menstrual irregularity, hirsutism, breast tenderness
Black box: tumors found in animals with spiranolactone
Patient teaching
Do not take K+ supplements
Recognize signs and symptoms of hyperkalemia (bradycardia, weakness, fatigue)
