Identify patients at risk of pressure injury measures to prevent pressure ulcer prevention

A

Being bedridden, unconscious, unable to sense pain, or unable to moveraises the risk that a pressure injury will develop. The risk goes up if you aren’t turned, positioned correctly, or given good nutrition and skin care.

Explain the mechanism that cause pressure injuries

A

Pressure ulcers are caused bya prolonged period of repeated friction and shearing pressure of the skin overlying the bony prominencesalong with some of the following intrinsic causes: Loss of skin fragility. Decreased blood flow. Loss of muscle volume.

1. Wash the eyelids, starting from the inside and moving out.
2. Wash the face, ears, and neck.
3. Wash the arms one at a time, and then the hands.
4. Wash the chest and belly, including the belly button.
