Estradiol and Norethindrone (Ortho Novum)
Estrogen-Progestin oral contraceptive
Inhibits LH, FSH
Can also treat acne, endometriosis, hypermenorrhea
If dose missed, take as soon as remembered or two the next day
DON’T TAKE IF PREGNANT
AE:
- Breast tenderness, nausea
- Weight gain
- Edema
- HTN/THROMBOEMBOLIC DISORDERS
Black box YOu will get fucked up if you smoke cigarettes with this
Orlistat for obesity
Lipase inhibitor
BMI of 30+
OR
27+ HTN, hyperlipidemia, DM
3 times daily during or prior to meal
Meal has to contain lipids
Keep at room temp
Should restrict fat intake to low/moderate amounts
AE:
- GI distress
- Can decrease fat soluble drug absorption and warfarin
Penicillin G
Antibacterial
15-30% of oral dose absorbed
Given often by IV or IM
Observe for allergic reactions for 30mins
Don’t mix penicillin and aminoglycosides in same IV- separate by 1 hour
AE:
- Diarrhea
- nausea
- Vomiting
- Pain at injection site
Late hypersensitivity can occur weeks into regimen
Diphenoxylate with atropine (Lomotil) for diarrhea
Antidiarrheal opioid
Slows peristalsis
Acts within 45-60min
13 and older
Atropine added to limit drug intake- causes anticholinergic effects
Measure drug accurately with dropper for children
Ondansetron (Zofran) for vomiting
Antiemetic
Start at least 30mins before chemo or radiation and for several days after
Administer to schedule, not PRN
Can prolong QT interval
Sulfasalazine (Azulfidine) for IBD
Treats ulcerative colitis
can treat RA as a DMARD
Inhibit inflammation in colon
Don’t admin to pt w/ allergies to sulfonamide antibiotics, aspirin, or furosemide (Lasix)
Don’t use under 2 yr old
AE:
- GI distress
- Dividing dose throughout day can reduce stress
- SJS
- Can impair male fertility
- Blood dyscrasia
Psyllium mucilloid (Metamucil)
Bulk laxative
Increases size of stool
Several doses may be needed over 1-3 days
Can cause small cholesterol drop (reduce risk heard disease)
Mix w/8oz. liquid, follow each dose with additional 8oz.
May swell in esophagus if not taken with enough water
Conjugated Estrogens (Cenestin) for HRT
Treat menopause, vaginal atrophy, pain during intercourse
Increase bone density, reduce LDL
Use calibrated dose applicator for vaginal cream
For IM and IV reconstitute by removing 5mL air from dry powder vial, then slowly inject dilutent aiming at side. Gently agitate to dissolve.
Admin at 5mg/min IV
AE:
Black box: Endometrial cancer in postmenopausal women. Can inc. risk of breast cancer w/medroxyprogesterone. Inc. risk of DVT, MI, dementia, pulmonary emboli
DON’T USE IF PREGNANT OR HAVE BREAST CANCER
Pancrelipase (Pancrease)
Pancreatic enzymes
Act locally in GI tract, not absorbed
Dose based off fat in diet
Give 1-2 hr before or with food
AE:
-GI irritation
Don’t give if allergic to pork
