Define Incident
Damage to Dept vehicle that’s properly parked or damage discovered during routine inspection of vehicle. Minor injuries, serious injuries, or fatal injuries to members while operating on parked rig.
Allegations as well.
Damage to civilian vehicle operating at alarms, MUD, & inspection activities
T/F Minor Accident: Minor to moderate damage to Dept/Civilian vehicle or private property. Any accident that results in no injuries to minor injuries only
False- NO injuries
Major Accident (2 “S”)
Significant damage & Serious Injuries/death
Can a rig be triple parked & still be considered properly parked
Yes at fires, emergencies, or false alarms unless investigating Chief Off feels member could have selected an alternate site less exposed
Actions by Off at accident: Preliminary report to Dispatcher “U- MR_ LIAR”
Unit involved, Major/Minor, Responding/Returning, Location, Injuries to civ/FF, Ambulance req'd, Remaining at scene or proceeding
T/F At Major accidents do NOT move the rig. However, if an accident occurs during periods of light or moderately fast moving traffic, the potential for another accident being caused is increased. The rig may then be moved

If the rig is moved from a major accident- what marks must be made
Location of tires of ALL vehicles involved marked with “T”. Tractor trailers & large vehicles (NOT rig) mark all 4 corners in addition to tires. Keep marks until arrival of Safety Battalion or Photo Unit
Additional forms req’d if an accident involves _ rigs, more than _ civilian vehicles, more than _ civilian pedestrians or bicyclists (NOT motorcyclists- motor vehicle), or more than _ people injured

T/F 2 or more rigs collide- each unit must fill out CD19. Properly parked rig always req’d to forward Apparatus Accident report whenever struck by another vehicle/rig

If two rigs collide- how many CD-19’s forwarded
At least four CD-19’s: 2 from each Officer. Officer of each unit considers their own rig as Vehicle 1 & other rig as Vehicle 2. Additional forms are req’d to ID rig & members of other unit
When is MV-104 req’d “1000 DAD”
1000 dollars/more damage to 1 car,
Death,
Any injury,
Dollar amount NOT determined
Can forwarding of Accident reports be delayed due to medical, vacation, regular, or other leaves
No: If Off unavailable, another Off assigned to unit shall prepare & forward report
Original MV-104 must be completed & sent to Dept of Motor Vehicles w/in _ days & copy sent to Safety Command- Accident Claims within _ hours

96 hours for “1000 DAD” to Safety Command
The Safety Command- Accident Claims Unit will notify an Off assigned to unit in which the rig was in an accident that an insurance agent will inspect the rig. If this occurs, what must OOD do?
Make an entry in Co Journal. Make sure appointment is 0800-1100 hours. Get name & number of insurance adjuster & his company
What if Unit gets an alarm during an insurance inspection
Inspection will cease. Adjuster shall reschedule the inspection
Can the adjustor move the rig
No: Have the Chauffeur move the rig
What time shall Fleet Maintenance On-site appraisals occur
0800-1600 hours. Only authorized Chauffeur shall move the rigs. Note: Rigs in fatal accidents shall be OOS immediately & a safety appraisal shall be made at scene
The following shall be considered Preventable Accident “Quentin Tarantino Directs Really Cool Films”
Quarters struck leaving/returning,
Tiller not operating tractor trailer,
Double parked car hit while in non-response mode or 10-20 mode,
Reverse gear when guide personnel are or should be available,
Chocked wheels- failure to do so,
Fall from rig
An Off involved in a 2nd preventable accident in a _ month period will result in Supervising Conference w/a Chief from the Off assigned Battalion. A 3rd preventable w/in _ month period Supervisory Conference w/Unit Cmdr & Chief from Unit Cmdr’s assigned Battalion plus Report to Ch of Ops w/recommendations for reassignment for _ day period

Apparatus accident- You inform Dispatcher that your company was involved in a minor accident and request to have PD and the Battalion respond. Next, you try to remember when you would need to fill out more than one CD-19 form. Incorrect:
A. More than one apparatus
B. More than one civilian vehicle
C. More than two civilian pedestrians or bicyclists
D. More than 3 people injured
D. More than four people injured. Safety Bulletin 56 page 8, 9.2
Remember “2235” You need two forms if: 2 apparatus, 2 civilian vehicles, 3 pedestrians or bicyclists, or 5 people injured.
MV-104 Incorrect:
A. MV-104 must be forwarded within 10 days to the NYS DMV
B. A copy must be filled out & sent to the Safety Command-Accident Claims Unit within 96 hours of an accident resulting in death, personal injury, or damage over $1,000.
C. If the dollar amount of the damage cannot be estimated, MV-104 must be completed and sent to the Safety Command Accident Claims Unit within 96 hours.
D. Completing the MV-104 is the express responsibility of the officer involved in the accident.

Who fills out a CD-19 and when does it have to be forwarded through the chain of command
The apparatus operator or Officer within 96 hours of the accident shall fwd via Ch of Command
