3 stages of secondary survey

History, vital signs, assessment. [in a loop]

Past medical history
Last meal
Events - including family and social history

Breathing rates:

Adult = 12-20
5-11 = 20-25
2-5 = 25-30
1-2 = 25-35
0-12 months = 30-40

Signs

What can you see, feel, hear?
(pulse rate, respiratory rate, blood pressure, LOC (AVPU), pupil reaction, skin (texture/colour/temperature), vomit)

Symptoms

What the patient tells you.
(palpitations, nausea, dizziness, pain)

Physical Assessment

head to toe survey:
head and face
neck
thorax (between head and abdomen)
abdomen
pelvis
extremities
spine
(check for medi-alert jewellery)

Head to Toe Examination

Visualise
Listen
Palpate (gently feel with hands/fingers)
Compare (symmetry)
Distal Pulses
Full movement all limbs

Examining Children

Consider foot to head to gain trust.
Never lie to a child - you'll loose their trust.
Consider demonstrating on a child's toy.
