Sensory Exam Flashcards
(23 cards)
Tools to assess vibration?
1128-Hz tuning fork
Apply both vibrating and non-vibrating fork to pt start ____________ and move ____________
Distally; proximally
UE locations (5)
- Distal phalanx of finger
- Ulnar styloid process
- Olecranon process
- Acromion process
- Cervical spinous process C7
LE locations (5)
- Distal phalanx great toe
- Medial malleolus
- Patella
- ASIS
- Lumbar spinous process
Where is the toe grasped for proprioception testing?
Minimal contact on sides of toe above DIP joint
How is proprioception of the toe performed?
Bilaterally, start with large ROM and work your way smaller. Start distal, work to proximal joints with impairment
What does the Romberg test test?
Proprioception
What is indicated if the pt is unable to perform test with eyes open?
cerebellum issues
What is indicated if the pt is unable to perform test with eyes closed?
proprioceptive issues
What is deep tendon reflex also known as?
Myotatic or muscle stretch reflexes
True or false: Deep Tendon Reflexes assess both the afferent and efferent components of spinal nerve
TRUE!
3 critical components of DTR:
- Patient must be relaxed
- Must be efficient using reflex hammer (assertive but not aggressive swing of hammer)
- Use reinforcement techniques if pt cannot relax (LE: Jendrassik maneuver; UE: squeeze knees together)
List the grades of deep tendon reflex
0. No response 1+ Minimal response 2+ Normal response 3+ Brisk response 4+ Hyperactive response (associated with clonus)
DTR C5 (muscle; stimulation sight; response)
Biceps; Biceps tendon; elbow flexion
DTR C6 (muscle; stimulation sight; response)
Brachioradialis; Brachioradialis tendon (prossima to radial styloid); elbow flexion
DTR C7 (muscle; stimulation sight; response)
Triceps; Triceps tendon (proximal to olecranon process); elbow extension
DTR L2,3,4 (muscle; stimulation sight; response)
Quadriceps; Patellar tendon (btwn patella & tibial tub); knee extension
DTR L5, S1 (muscle; stimulation sight; response)
Achilles, achilles tendon; ankle plantar flexion
Do cutaneous reflexes asses UMN or LMN lesions?
UMN lesions (lesions of the neural pathway that are above the anterior horn cell OR motor nuclei of the cranial n.
What does absence of abdominal reflex indicate?
Lesion of UMN from T6 to L1 depending on level of absence
What is a negative Babinski? Positive?
Negative- toes curl in
Positive- toes splay out
What is a negative Hoffman? Positive?
Negative- no reaction to stimulus
Positive- Reflex flexion of distal phalanx of thumb and index finger
What are the 5 elements of coordination test?
- Heel to shin
- Heel to contralateral knee
- Finger to nose
- Rapid alternating pronation/ supination of wrist
- Matching to finger position
