What are some key terms in discussing social issues?
- Health literacy
- Bolstering self efficacy
- Use of new technologies, including email, phone applications, text messaging
- Engaging community organizations
- Engaging network of key supports
- Negotiation of numerous conflicting interests
Describe poor insight in term of the three key questions for social issues?
1) Why is it important?
2) Why is it happening?
3) How can it be addressed?

Why is it important?
- Increases compliance, fosters self efficacy
Why is it happening?
- Is there underlying dementia or depression? Has no one taken the time to explain it to the patient?
- Address the Bio, psycho, social and functional issues underpinning it
How can it be addressed?
- Ensure not due to secondary pathology (ie dementia or depression)
- Dedicated education (with different modalities of education ie websites, phone number), make sure pt has documentation of health conditions medications etc
- Brief interventions framework
- Community groups
- Utilise support network (use family members and local doctors

Why is it important?
- Patient enjoyment, QoL, mortality, mood/mental health disorders
Why is it happening?
- Recognize and investigate causative, contributing and exacerbating factors
- Recognize and investigate barriers to change (bio, psych, social, functional) ie functional limitation means can go outside much
How to address?
- Ensure not secondary to underlying pathology (ie dementia or depression)
- Improve access (community transport), and address functional limitation (PT, OT input, walking aids)
- Link in with community groups (mens shed, community day centers, RSL clubs, church groups if religious, local bingo groups)
- Bolstering support networks - Encouraging discussion with family / friends re issues, pets / support animals

- Reduced access to medical services
- Increased costs of transport to medical services
- Reduced timely emergency care
- Increased rates of mental health issues and social isolation
Why is it happening?

How can it be addressed?
- Use of new technologies (emails, skype, teleconferencing)
- Co-ordinatre specialist apts to occurs on the same day or in close succession
- Engage outreach services
- Address mobility and transport issues
- Be realistic with the patient

- Barrier to accessing medical services (OPC, meds, therapies, AH etc)
- Reduced access to food and services
Why is it happening?

How can it be addressed?
- DSP (if applicable), centerlink job seeker payments (need to be able to make applications), Aged care pension, Housing assistance, sickness benefits

1) Why is it important?
2) Why is it happening?
3) How can it be addressed?

Why is it important?
- Directly and indirectly lead to poor outcomes (increased morbidity and mortality, increased complications), disruption of medical relationship (ie multiple FTA or consistent non compliance)
Why is it happening? - bio, psycho, social, funcitonal
- No percieved health benefit (variable insight)
- Cost
- Side effects
- Difficult formulaiton (too big tablets), and dosing (to many tablets)
- Busy lifestyle, social barriers
How can it be addressed?
- Foster good patient rapport, build a trust based relationship. Often need to compromise
- Single tablets (combination medications)
- Daily dosing medications
- Address polypharmacy / rationalise meds
- Webster packs
- Engage community pharmacist
- Address and investigate side effects, try different medications if SE’s
