A
  1. LMWH for atleast 5 days or INR > 2 for 24+ hours
  2. VKA for 24 hours and continued for 3 months
  3. Assess risks and benefits of continuing anticoag for VTE prevention

  1. LWMH for 6 months

Mx of confirmed PE or proximal DVT

A
  1. Offer LWMH or fondaparinux asap unless
    - severe renal impairment
    - increased risk of bleeding
    - haemodynamically unstable
  2. Offer a VKA (e.g. warfarin) to patients with confirmed proximal DVT or PE within 24 hours of diagnosis and continue warfarin for atleast 3 months
