Mesalazine, Sulfasalazine

A

BRANDS: Asacol, Octasa, Pentasa
CLASS: Aminosalicylates

INDICATIONS: Both used for Ulcerative Colitis (M 1st line) and Crohn’s disease. Both are severe inflammatory bowel diseases. S used in rheumatoid arthritis also.

ACTION: Release 5-amimosalicylic acid (5-ASA) which has anti-inflammatory and immunosuppressive effects. Acts locally on gut and not systemically.

S/E: M causes less than S. M causes GI Upset and Headaches. S causes decrease in sperm count and hypersensitivity. Both cause BLOOD ABNORMALITIES and renal impairment.

WARNING: Allergy to aspirin (same class)

INTERACTIONS: pH sensitive coating brands e.g. Asacol MR. PPIs and Lactulose affect gut pH

COUNSELLING: Foam enema, suppositories, m-r tablets all different administration. Keep same brands. Test for blood abnormalities, FULL BLOOD COUNT. Renal and Hepatic monitoring for damage
