Ethnicity?
sickle cell affects mainly west African and Afro-Caribbean patients.
Thalassaemias and G6PDH deficiency affect Mediterranean, east Asian and African.
are they pregnant?
Intrahepatic cholestasis of pregnancy, pre-eclampsia with HELLP syndrome or acute fatty liver of pregnancy.
RUQ discomfort, nausea, vomiting, pruritus?
hepatitis
Fever or diarrhoea?
May be present if liver is infected.
Steatorrhoea, dark urine and pruritus?
obstruction to biliary flow.
weight loss, fever, night sweats?
could indicate a malignancy of the liver, bile duct or pancreas.
bronzed skin, signs of DM?
haemochromatosis.
exposure to outdoor water / sewage?
Risk factor for leptospirosis.
PMH drugs that cause intravascular haemolysis?
sulphonamides, aspirin in G6PDH patients
PMH drug that causes autoimmune extravascular haemolysis?
methyldopa
PMH drugs that cause hepatitis?
paracetamol OD
PMH drugs that cause cholestasis?
co-amoxiclav
family history?
Gilberts syndrome, haemochromatosis, Wilsons disease, sickle cell disease, thalassaemia, hereditary spherocytosis, G6PDH deficiency
social history?
Alcohol consumption? IV drug use? Unprotected sex? Foreign travel? Tattoos?
