Elevated PT Flashcards

1
Q

Which factors are measured using PT

A

assesses extrinsic pathway factors and common pathway factors,

therefore, 2, 5, 10, 7, and fibrinogen.

Q

Pt is measured in ____ as the amount of time required for the formation of.a ___ ____ after the addition of ____ or __ ___ to the plasma.

A

Pt is measured in SECONDS as the amount of time required for the formation of.a FIBRIN CLOT after the addition of THROMBOPLASTIN or TISSUE FACTOR to the plasma.

THE PT IS NOT SENSITIVE TO DEFICINECIES OF COAGULATION FACTORS CONTAINED WITHIN THE INTRINSIC PATHWAY

Q

PT can be elevated as a result of congenital ____ deficiency.

A

congenital factor 7 deficiency. Bleeding symptoms will also be seen along with increased INR if the factor 7 activity level is less than 10%.

Q

how can congenital factor 7 deficiency be managed to normalize INR

A

bleeding can be managed with activated recombinant factor 7 replacement therapy.

Q

Acquired deficiencies of factor 7 such as:

may elevated INR

A

may be sue to 1. vitamin K deficiency, liver disease, or warfarin therapy.

vitamin K, congenital factor deficiencies, and liver disease would most likely also affect PTT too, because they are responsible for making coagulation factors of the intrinsic pathway as well.

Q

why may vitamin K deficiency cause PT elevation

A

it prevents the synthesis of factor 7 (extrinsic pathway), which has a very short half life so you’d probably see it immediately as soon as you become vitamin K deficient.

  • vitamin K is needed to carboxylate factors 5, 7, 9. and 10. Vitamin K also helps to synthesize protein C and S to promote their anticoagulant activity.

Q

Why may liver disease cause PT elevation

A

liver synthesizes all clotting factors besides factor 8 and VWF (megakaryocytic and endothelial cells). therefore, its dysfunction would prevent clotting.

Q

why may warfarin cause PT elevation

A

warfarin is a vitamin K inhibitor. prevents it from carboxylated factors 5, 7, 9, 10 and protein C and S. Also gotta calculate INR for these patients.
