What test is used to determine the cause of ascites

A

serum ascites albumin gradient

  • serum [albumin] - ascitic fluid [albumin]

High SAAG > 11g/dL = raised portal pressure

Low SAAG < 11g/dL

  • pancreatitis
  • peritoneal cancer
  • TB
  • Nephrotic syndrome

How do you manage ascites

A
  • Treat underlying cause
  • Salt restricted diet
  • Fluid restriction
  • Spironolactone
    • Adjunctive diuretic therapy (e.g furosemide) if unresponsive.
  • ascites refractory to medical management –> regular therapeutic paracentesis
