What test is used to determine the cause of ascites
A
serum ascites albumin gradient
- serum [albumin] - ascitic fluid [albumin]
High SAAG > 11g/dL = raised portal pressure
Low SAAG < 11g/dL
- pancreatitis
- peritoneal cancer
- TB
- Nephrotic syndrome

How do you manage ascites
A
- Treat underlying cause
- Salt restricted diet
- Fluid restriction
- Spironolactone
- Adjunctive diuretic therapy (e.g furosemide) if unresponsive.
- ascites refractory to medical management –> regular therapeutic paracentesis
