What is Acute limbischemia(ALI)?
is a vascular emergency in which the arterial blood supply to one or more extremities is critically reduced.
What is responsible for the majority of cases of acute limb ischemia?
Arterialthrombosisandcardiac emboliare responsible for the majority of cases.
What does limb hypoperfusion leads to?
Limb hypoperfusion leads to systemic acid base and electrolytes abnormalities that impair cardiopulmonary and renal function.
What are the clinical features of acute limb ischemia?
Clinical features: “ 6 Ps”
- Pain: Restpainthat worsens on passive movement of the limb and is most severe in thedistalaspects of theischemiclimb.
- Pallor: Skin over theischemiclimb initially appears pale and then becomes mottled andpurple-blue.
- Paralysis: Initial muscle weakness progresses to profound irreversibleparalysis.
- Pulselessness: Absent peripheral pulsesdistalto the site of occlusion.
- Paresthesia: Initialparesthesia(e.g., decreased fine touch sensation) progresses to anesthesia.
- Poikolothermia: Theischemiclimb is typically cold to touch but may be warm in hot environments (i.e., it takes on the ambient temperature).
What is used to categorize the limb?
Clinical findings in combination with Doppler studies are then used to categorize the limb as viable, threatened, or nonviable.
