Class 1 Antiarrhythmics

Lidocaine
Class 1B
Blocks Na Channels to increase threshold
Slows rate of phase 0
Use dependent
Ventricular arrhythmias
Sotalol
Class 3 (K+ blocker) Phase 3: delays repolarization and lengthens APD
Amiodarone
Class 3 (most common drug) (K+ blocker) Phase 3: delays repolarization and lengthens APD
Ventricular arrhythmias, atrial fibrillation/flutter.
SE because of iodine, risk of TdP
Dronedarone
Class 3 (trial phase) (K+ blocker) Phase 3: delays repolarization and lengthens APD
Less toxicity than amiodarone.
Severe VT/VF.
Propafenone
Class 1C
Beta-Blockers
Class 2
Negative chronotropic and ionotropic effects
Adjunctive
Contraindicated: Acute heart failure, asthma, arrhythmias with AV block
Adenosine
Like ACh
Decreases sinus rate and AV conduction, anti-ischemic.
Used for supraventricular tachy.
Digoxin
Increased vagal tone -> slows AV conduction
Occasionally for A fib, increases contractility.
Toxic beyond tiny therapeutic window.
