Appendix by Dra. Turingan Flashcards
How can you locate the appendix?
find the taenia colie. Base of appendic is located following longitudinally oriented teania coli. Tip of appendix is located anywhere in the RLQ, ABD, pelvis, retroperitoneum
the appendix is located onthe LLQ in this rare autosomal Recessive condition
Situs inversus
1st diagnostic imaging ordered for an adult suspicious for AP
CT
1st diagnostic imaging of choice for children suspicious for AP
USG
what microorganism causes gangrenous perforated AP
Bacteroides
most common microorganism causing inflamed AP
E.Coli
Symptoms that mostly point to AP
Periumbilical ad diffuse pain that eventually localizes to RLQ
What is the Alvarado score?
<3- low likelihood of AO, 4-6 t/c more imaging, >7 highly likeli AP
Alvarado score for tenderness R iliac fossa, Shift to left of neut, migratory R iliac fossa pain, Leukocytosis >/= 10x10^9 cells/L
2-Alvarado score for tenderness R iliac fossa , 1-Shift to left of neut, 1-migratory R iliac fossa pain, 2-Leukocytosis >/= 10x10^9 cells/L
Describe the scoring of AP inflammatory response score
0-4 low probability, out patient follow up, 5-8 indeterminate grp, active observation of diagnostic ap, 9-12 high probability, surgical exploration
Differentiate open AP, and lap AP
Open AP- decrease operative duration, surg site infection, LAP AP- less pain, shorter length of stay, Risk-intraABD abscess, increased cost
Most common site of SSI in LAP AP
extraction port site
procedure for malrotation d/t displacement of cecum to the LLQ complicating the dx of AP
LADD’s procedure
MGT if tumor size is = to 1cm
appenddectomy
Indications for RIGHT hemicolectomy
tumor is 1-2cm located: base/ mesenteric involvement/ LN mets
