16
Q

Explain: “All Physicians Eat Tender Meat”

A
Aortic --> R 2nd ICS -- S2
Pulmonic --> L 2nd ICS -- S2
Erb's --> L 3rd ICS
Tricuspid --> 4th ICS -- S1
Mitral --> 5th ICS mid clavicular -- S1
17
Q

What causes physiologic splitting of S2?

A

On inspiration, increased pressure causes more blood to enter from vena cavae and rt. atrium making the pulmonic valve stay open longer.

18
Q

Xanthelasma is?

A

waxy fat deposits from hypercholesterolemia

19
Q

What is the pericardium?

A

Fibrous sac encasing the heart

20
Q

Precordium?

A

Area of the chest overlaying the heart.

21
Q

Systole

A

ventricles are contracting

22
Q

Diastole

A

Ventricles filling w/ blood

23
Q

What is pathological splitting?

A

When there is a split between aortic and pulmonary valves regardless of inspiration of expiration, or when the aortic and pulmonary valves are split wide on expiration and are closer during inspiration

24
Q

What is preload?

A

The amount the ventricles stretch out due to blood prior to contraction

25
Q

What is after load?

A

What the ventricles have to work against to pump out blood during contraction - forces that oppose ejection of blood

26
Q

Gallop

A

either s3 or s4, or both

27
Q

When does s3 arise?

A

Mitral regurgitation

28
Q

When does s4 arise?

A

During the atrial “kick”

29
Q

What is a rub?

A

When the heart rubs against the pericardium, is generally continuous throughout s1 and s2. Sounds like creaking leather.

30
Q

What is a click?

A

A adventitious sound due to mitral valve prolapse

31
Q

What is a snap?

A

Adventitious sound due to mitral valve stenosis.

32
Q

What are ejection sounds?

A

Adventitious sounds due to aortic valve stenosis.

33
Q

What is the electrical current route in the heart?

A

SA node, AV node, Bundle of His, Purkinje Fibers, then contraction starts at the apex of the heart and goes north.

34
Q

Claudication

A

Cramping pain in the leg induced by exercise, typically caused by obstruction in the arteries

35
Q

Intensity of a heart murmur goes from 1-6. In which level does a palpable thrill become apparent?

A

Level 4 out of 6
