A
Chest symptoms - SOB, orthopnea, PND Volume status: - Distended jugular veins - more prominent on inspiration, increased JVP - Pulmonary congestion - Peripheral edema (+/- ascites) Palpation - right ventricular heave - right sternal border with severe TR - tachycardic - palpatations +/- lightheadedness - irregularly, irregular pulse - pulses alternans ?? Hepatomegaly - RUQ discomfort

Class III/IV angina
Baseline digoxin, diuretic, ACEi and betablocker

- value does not stabilize in 2-4 weeks
