Stupor
eyes open brief w/ stimulation along w/ cognitive dysfunction
Coma
eyes closed w/ stimulation
Delirium
disoriented, misperception, hallucinations
Post coma: Abulia
Associated disease
awake but apathetic w/no spontaneity;
cognitive fxn normal w/ vigorous stim
Dz: bilateral frontal lobe / lobotomy
Post coma: akinetic mutism
Assoc. disease
silent, alert appearing immobility w/ no increase mental activity and vigorous stimulation
Dz: frontal lobes / hypothalamus
Post coma: Minimally Conscious state
Example
fragments of awareness & meaningful interaction;
Patient reaches and grunts to communicate
Post coma: Vegetative state
awake w/ no awareness of meaningful interaction w/ environment
Brain maintaining vegetative fxn (HR, BP, ANS)
Cheyne Stokes respirations
When do you see them?
increase apnea w/ intermittent hyperventilation seen in both central & transtentorial herniations
