Increased neutrophil elastase results in pulmonary injury from degradation of elastin–> leads to emphysema

Insidious onset
Fever, malaise, weight loss
Wegener Granulomatosis
Peripheral eosinophilia
Rash

Churg-Strauss Syndrome
- Chronic, productive cough
- Exertional Dyspnea, easy fatiguablity
- Rt ventricular systolic and diastolic failure from lung disease or pulmonary vascular disease
Cor Pulmonale
Profound dyspnea 12-48hrs after precipitating event
Severe lung condition that prevents enough O2 getting from lungs into blood
Acute Respiratory Distress Syndrome
SOB
Dyspnea
Decreased lung sounds
Barrel chest
Emphysema
Severe reduction in venous return from head, neck and upper extremities
Face/neck swelling
Periorbital edema, dypsnea, JVD
Superior Vena Cava Syndrome
Localized sharp, sharp, fleeting pain->worse with cough, sneeze, deep breaths
Pain may refer to shoulder
Most commonly viral infx
Pleuritis
Chronic, recurrent, productive cough
Dyspnea, wheezing Clubbing
Increased A/P diameter
Apical crackles
Hereditary
Cystic Fibrosis
Chronic productive cough
Dyspnea, wheezing
Basilar crackles
Dilated, thickened airways w/ scattered irregular opacities on CXR
Cystic fibrosis causes 50% of cases
Bronchiectasis
Excessive fluid accumulates in pleural cavity
May be asymptomatic to CP, dyspnea, Dullness to percussion and decreased breath sounds
Pleural Effusion
Fever/Chills
Tachypnea, dyspnea
Cough w or w/o sputum
Bronchial breath sounds or inspiratory crackles
Typical pneumonia
Wheezing, chest tightness SOB, cough
Excess sputum production
Asthma
Pleural effusion 2/2 to pneumonia, lung abscess or bonchiectasis
Paraneumonic effusion
