Avoid mitigating language (ie. Lets intubate not we might need to think about intubation)
Below 10 (sterile cockpit, below 10,000 rule)
Reasons for failure- noise, hierarchy, jargon, cognitive load/ focus

Manage a colleague who may be about to make a drug error, how would you approach this

A

Probe- What is the dose supposed to be again?
Alert- Can we recheck the dose on that to make sure its correct
Challenge- That is the wrong dose, we need to check this again
Emergency- STOP, do not give that drug, it is the wrong dose.
