What is dyspepsia
- Describes a group of symptos
Upper abdominal discomfort, reterosternal pain, anorexia, nausea, pain vomiting, fullness, early satiey
What are causes of Upper GI dyspepsia
- Peptic ulcer
- Gastritis
- ## Gastric cancer
What are the ALARM features?
- Anorexia
- Lose of weight
- Anemia - Iron deficiency
- Recent onset > 55
- Melena/Heamatemesis
- Swallowing problems - dysphagia
When would you do and upper GI if the patient didn’t present with alarm features
- Test for h-pylori infection
Describe the Helicobacter pylori bacteria
Where can H pylori colonise
- The stomach and only the stomach
- Resides in the surface mucousal layer and does not penetrate the epithelial layer
What are the potential clinical outcomes of H.pylori disease
- Gastric ulcers
- Peptic ulcers
- Gastritis
- Gastric cancer
What is the diagnosis for H.pylori?
- Non-Invasive: 13/14c Urea breath test
- Serology IgG antigen test
- Invasive: Endoscopy
What is the more invasive therapy to diagnose H.pylori infection?
- Endoscopy
- Rapid slide urease test (CLO)
What are the three different cause’s of gastritis?
- Autoimmune (Parietal cells)
- Bacterial (H pylori)
- Chemical (bile/NSAID’s)
What are the main causes of peptic ulcers, who does it effect more?
H pylroi, Men more than women
