WHAT IS IMMEDIATE IMPLANT PLACEMENT?

A

Immediate implant placement is when an implant is placed at the same time as the natural tooth is extracted .

  • Implant placement in fresh sockets was first reported by Schulte and Heimke in 1976, termed ‘immediate implant’.
  • Over the years there have been many studies reporting high survival rates.
  • The expert recommendations published for single implant placement following the Foundation for Oral Rehabilitation (FOR),were that careful patient selection , proper treatment planning , and precise implant placement were necessary to gain predictable and aesthetic results .

The ideal extraction site for immidiate implant placement

A
  • The ideal extraction site for immediate implant placement is one with little or no periodontal bone loss on the tooth that is to be extracted, such as a tooth being extracted due to endodontic involvement, root fracture, root resorption, periapical pathology, root perforation, or unfavourable crown-to-root ratio
  • Most reasearch recommend at least 3 to 5 mm of bone beyond the apex and a bony length of 10 mm or greater for stability when placing immediate implants

CONTRAINDICATIONS

There is general consensus that suitable sites for immediate implant placement do not include those with:

  • severe labial
  • circumferential bone defects or
  • bony defects with two or three missing walls
